Scout drinks a lot of water. Always has. She is a Border Collie with a high metabolism and an active lifestyle. Her water bowl empties twice a day. I refill it without thinking.
Then one day, the bowl sat full for 24 hours. I did not notice at first. I was busy. I was tired. I assumed she had drunk from it when I was not looking. But by day 2, the water level had barely dropped. And Scout seemed... off. Less energetic. Slightly withdrawn. Sleeping more than usual.
I almost dismissed it. Dogs have off days. Maybe the weather was affecting her. Maybe she had found a puddle outside and was drinking from that instead. Maybe I was being paranoid.
I brought her in for blood work because I am a vet and I have access to blood work and because something felt wrong. The results: early-stage chronic kidney disease. Her kidneys were not concentrating urine properly, which meant she was not feeling thirsty even though she needed hydration. The reduced drinking was a symptom, not a choice.
Why Water Intake Changes Matter
Water intake is one of the most underrated health indicators in dogs. Changes in drinking behavior — either more or less — are often the first sign of an underlying medical issue.
Increased drinking (polydipsia): Can indicate diabetes, kidney disease, Cushing's disease, liver disease, or pyometra. Any significant increase in water consumption warrants a vet visit.
Decreased drinking: Can indicate kidney disease, nausea, mouth pain, fever, or neurological issues. It can also lead to dehydration, which compounds whatever the underlying problem is.
Variable drinking: Some days lots, some days none. Can indicate intermittent nausea, pain, or behavioral issues.
The key is knowing your dog's baseline. How much do they normally drink? How often do you refill the bowl? If that pattern changes for more than 2-3 days, pay attention.
The Kidney Connection
In Scout's case, the decreased drinking was paradoxical. Kidney disease usually causes increased drinking because the kidneys cannot concentrate urine, leading to excessive urination and compensatory thirst. But in early stages, some dogs actually drink less because the kidneys are not sending proper thirst signals to the brain.
Her blood work showed elevated BUN and creatinine — not dramatically, but enough to indicate reduced kidney function. Her urine specific gravity was low, confirming that her kidneys were not concentrating urine effectively.
The diagnosis: IRIS Stage 1 chronic kidney disease. Early. Manageable. Caught because I noticed a water bowl.
Management
Scout's treatment plan is straightforward:
Hydration support: We add water to her food. We offer bone broth. We use a pet water fountain, which encourages drinking through movement and freshness. We monitor her water intake daily.
Diet: A renal-support diet with moderate protein, restricted phosphorus, and added omega-3s. The goal is to reduce the workload on her kidneys while maintaining muscle mass.
Supplements: Azodyl (probiotic for nitrogenous waste) and omega-3 fatty acids for kidney support.
Monitoring: Blood work and urinalysis every 3 months. Weight checks monthly. Water intake tracking daily.
What I Tell Clients Now
I now ask every client about water intake at every visit. Not just "is he drinking okay?" but "how many times do you refill the bowl per day?" "Has that changed?" "When did it change?"
Most owners do not track water intake. They should. It takes 10 seconds to notice the bowl is empty or full. Those 10 seconds can catch kidney disease, diabetes, or Cushing's months before other symptoms appear.
I also recommend water fountains for senior dogs. The moving water is more appealing. The filtration keeps it fresh. And the reservoir makes it easy to see if intake has changed — just check the water level.
The Lesson
Scout is doing well. Her kidney values have stabilized. Her energy is back. She drinks from the fountain regularly. And I check her water level every morning like a ritual.
Small observations save lives. A full water bowl. An empty water bowl. A change in pattern. These are not nothing. These are data points. And in veterinary medicine, data points are everything.
Pay attention to the bowl. It is telling you more than you think.
